REQUEST FOR INFORMATION PATIENT CASE MANAGER (PCM) DEPARTMENT OF VETERANS AFFAIRS Description The Department of Veterans Affairs (VA) in support of the Veteran Health Administration (VHA), Atlanta VA Health Care System (AVAHCS) clinical and community care is seeking information on contractors interested in and capable of supporting its requirements for brand name Document Storage Systems (DSS) Patient Case Manager (PCM) (consisting of PCM Suite, PCM Analytics, Consult Tracking Manager Plus (CTM Plus) with Ad hoc Premium Application Workflow Support) software. The North American Industry Classification System (NAICS) for this requirement is 541519. THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes ONLY. Do not submit a proposal.
This RFI is for planning purposes only and shall not be considered an Invitation for Bid, Request for Task Execution Plan, Request for Quotation, or a Request for Proposal. Additionally, there is no obligation on the part of the Government to acquire any products or services described in this RFI. Your response to this RFI will be treated only as information for the Government to consider. Respondents are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party s expense. Not responding to this RFI does not preclude participation in any future solicitation, if any is issued. Background Patient Case Manager (PCM) is a platform designed specifically to support healthcare provider organizations on the journey to high reliability. This software application integrates with the facility s electronic health records Veterans Health Information Systems and Technology Architecture/Computer Computerized Patient Record System (VistA/CPRS) system and can pull information in real time helping the facility achieve, track, and monitor their compliance with the administrative and clinical business rules.
Case management is a complicated task that has significant impact on care delivery to our Veterans. Ensuring Veterans are scheduled, and care is coordinated for consults enter into the Atlanta VA Health Care System (AVAHCS) clinical and community care services is a monumental task. PCM provides many benefits to improve case management and help to streamline care for our Veterans. PCM delivers actionable patient information and medical knowledge into the workflow of clinicians at the point of care. This enables care givers to intervene faster, reduce length of stay, and efficiently provide better and safer care. Better decisions alter the healthcare experience and outcome for each patient, while reducing costs. The most important features that AVAHCS will receive with PCM is patient safety and quality of care. Both of these two features are achieved through clinicians receiving the right information at the right time about the right patient, so that they can make better and more-timely decisions.
Use of PCM will not only improve patient s outcomes but will also improve the experience for the patients and their families. PCM will provide more meaningful, timely and considered documentation to improve communication between providers. Along with PCM, VA requires the Consult Tracking Manager Plus (CTM+) software. CTM+ is a web-based software solution that provides an instant, up to date dashboard view of the status of all patient consults in a clinical environment. Consult Tracking Manager is a server-based software that integrates with clinical data within VistA using a Remote Procedure Call (RPC) Broker interface connection and `serves` a Graphic User Interface (GUI)/Web application that is accessed through a client web browser. This technology does not store data. Data accessed is stored in within VistA. AVAHCS is comprised of one VA Medical Center (VAMC) and 19 VA clinics. AVAHCS locations can be found at https://www.va.gov/atlanta-health-care/locations/. Scope The Contractor shall provide brand name DSS PCM (consisting of PCM Suite, PCM Analytics, CTM Plus with Ad hoc Premium Application Workflow Support) software. The Contractor shall provide installation and training of the PCM software at the AVAHCSi. The Contractor shall provide support services to aid and support as needed when problems occur with the software.
The Contractor shall provide all resources necessary to accomplish the deliverables described in this Product Description (PD), except as may otherwise be specified.
