…DUTIES:
- Provide ongoing professional assessment, coordination, planning, and advocacy on behalf of the Veteran to ensure necessary clinical services are received in a timely and cost-effective manner.
- Demonstrates knowledge of health informatics, i.e., design, configuration and management of health information and clinical support systems, health data collection, reporting, and communications.
- Oversees informatic practices, establishing effective clinical and administrative systems, designing useful decision support systems, managing and delivering educational experiences, enhancing lifelong learning, and supporting healthcare research.
- Focuses on application of informatics and information technology to deliver healthcare services, and develops business requirements, workflows, and processes specifically in support of health information systems.
- Responsibility and accountability for integrated program crossing services and/or discipline lines
- Acquires and provides guidance on VHA guidelines, utilization review criteria, current evidence-based standards of care
- Responsible for the documented outcomes at the program or service level.
- Actively leads groups and takes the lead in practice changes, is active in the facility nursing community and beyond, sought out for expertise, and actively volunteers and participates on committees.
- Leads and organizes delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning
- Guides, develops, and supports staff from a leadership perspective.
- Uses professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
- Provides direct patient care at least 10-25% of their time.
- Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision making, and evaluating outcomes.
