“The contractor shall be responsible for performing all tasks necessary to draw patient samples, design survey tools, produce surveys, field surveys, receive returns, retrieve and collect data, analyze results, and produce reports. The core survey related tasks and services shall include the following:
- Create, produce and execute analytical programs for: patient sample list creation from sample frames; de-duplication across recent and concurrent samples; tracking of “do not survey/no publicity” and deceased patients; data collection; results analysis; results reporting and trending.
- Sample list cleaning (bad addresses, incomplete records, etc.) and National Change of Address (NCOA) cleaning
- Survey material development and production (electronic survey invitation/printed pre-notification letter, electronic and paper cover letters, electronic and printed questionnaires, envelopes, postcard/email reminder) design and printing
- Survey ID number assignment report production
- Invitation/pre-notification and survey emailing/mailing
- Survey returns receipt and processing
- Survey data collection
