“The VA Butler Health Care Center, located at 353 North Duffy Road, Butler, Pennsylvania, is currently utilizing Telecare Record Manager (TRM) Plus with a combined clinical triage component to provide telephone triage services for all VISN 04 patients. Veterans Integrated Service Networks (VISN) 04 is made up of (9) Veterans Affairs Medical Centers (VAMC) – Altoona, Butler, Coatesville, Erie, Lebanon, Philadelphia, Pittsburgh, Wilkes-Barre, Wilmington.”

“This purpose of this contract is to provide DSS TRM Plus with the combined clinical triage component license to the Butler Call Center and VISN 4 offices. The application supports nurse clinical decision-making by systematically assessing a veteran’s symptoms while providing reference health information during the process. At the end of the triage process, the software provides a triage follow-up recommendation, self-care, or interim-care instructions. The systems sort patients into six (6) distinct urgency levels: Now 911, Now 2-8 hours, 12-24 hours, 2-3 days, 1-2 weeks, and self-care. The encounter documentation is automatically performed in the background during the triage process and saved as a triage encounter record. These applications have been shown to appropriately identify emergencies, while reducing unnecessary clinic visits…”
